Frequently Asked Questions
What is Direct Primary Care?

Direct Primary Care (DPC) is a healthcare model focused on maximum value for both patient and provider. You establish a direct agreement with your provider, which ensures no other parties can dictate the terms of your care. For a flat monthly fee, you retain Samuel Mann, ARNP as your personal Clinician and are guaranteed all of the services in our Agreement. With us, you will get on-demand, trustworthy, and effective healthcare.

Do You Accept Insurance?

No, we do not accept or bill insurance in our direct primary care practice. We recommend you have insurance coverage for catastrophic events like surgeries, trauma, serious illness, and hospitalizations. Health insurance helps pay for labs, radiology, specialist visits, and hospital care. Many patients who join us opt for a high-deductible health insurance plan which can provide significant overall cost savings to the patient. Also, many people never reach their deductible, and end up paying for most of their care out-of-pocket. For these people, a model like ours can end up saving them a significant amount of money.

Why DPC vs. Traditional Insurance-Based Primary Care?

The Direct Primary Care model has benefits for both patients and clinicians. It is, in many ways, a return to how healthcare was practiced originally, before insurance companies got in the middle, and before corporations and venture capital took the heart and soul out of medicine.

For patients, immediate benefits include:

  • Quick and timely appointments, in-person or virtually, to address your needs.
  • Expansive and comprehensive visits that can address all of your concerns, not just one at a time.
  • You can call, email, or text your clinician and receive a prompt response. No automated phone trees to navigate.
  • No surprise billing. All of our services are included in the flat monthly membership fee.
  • A real professional relationship with me, your Clinician, where we will establish a deeper level of trust, understanding, and support than most other mainstream practices could provide.

Imagine, too, the long-term benefits of having a Clinician who you know you can trust. Someone you can call when the unexpected happens. Someone who knows you. Someone who can advocate for you in this convoluted mess we call the American Healthcare System.

For me, as a Healthcare Professional, I hope to realize benefits too, including:

  • The ability to practice with Dignity and Integrity, in alignment with my values.
  • Limiting my practice to around 400 patients (versus 2,000+ in traditional primary care models), which allows me the time to go deeper and do more for my patients.
  • Bringing Joy back into the work I do, which will prevent burnout (a major problem affecting many clinicians today)
  • Less overhead and paperwork. Straightforward billing: no need to hire someone just to be able to bill insurance.
  • A true sense of ownership in my practice.
What Does the Monthly Fee Include?

When you are a Member of Mind and Matter Health, you can expect to receive these services. For full details, please read Appendix 1 – Services Provided in your Patient Agreement.

  • Initial Comprehensive Physical – A initial, in‑person, intake examination to establish your baseline health information and a plan of care.
  • Preventive & Wellness Visits– Periodic preventive visits and annual physical exams, routine check‑ups, and healthcare planning tailored to Your health.
  • Unlimited Follow‑Up Visits – An unlimited quantity of follow-up visits for certain acute or chronic issues, if appropriate for the chosen modality. In-person visits are subject to the Provider’s schedule and availability.
  • Communication & Access – Direct 24/6 access (Monday–Saturday) to Samuel Mann, ARNP via secure messaging or phone. Messages sent on Sundays will be answered the next business day.
  • Lab & Imaging Orders – Ordering and interpretation of necessary labs or imaging. External lab or imaging costs are the Patient’s responsibility. Cash-only prices may be available and pre-negotiated by the Practice.
  • Specialist Referrals – Coordination and referral to specialists when necessary.
  • Forms & Paperwork – Completion of standard medical forms such as FMLA, sports physicals, and pre-employment exams.
What About Labs, X-Rays, etc.?

I am able to order labs and imaging to various vendors in the region, including Quest Diagnostics and LabCorp. I send a requisition order and then you are responsible for traveling to a collection site (or having them come to you – see Quest Mobile)

You will be responsible for the costs of labs, imaging, and any other services rendered by another company or vendor. However, I have negotiated cash prices with these vendors, which are significantly cheaper than billing insurance. You may, of course, decide to use your insurance with these vendors.

Can I Use HSA or FSA Funds to Pay the Monthly Fee?

Yes! As of January 1st, 2026 you may use your HSA or FSA account to pay for the membership fee. It may be possible to use these accounts to pay in 2025, but I recommend you double check with your insurance plan.

What Sort of Coaching Do You Offer?

I want to offer my clients the ability to go deeper on issues commonly overlooked that have significant impacts on their health. These issues can range widely, from hidden addictions, self-esteem and relationship issues, traumatic childhood experiences, and more. A lot of men just go through life, gritting their teeth and enduring a lot of hardships, without really talking about it or reflecting on how it affects them. I want to offer my clients the ability to talk, Mann to Man™.

Mann to Man™ is a supplemental, non-medical service that I offer in addition to the Direct Primary Care Membership. For members, it is only $75 for a 50-minute coaching session. Coaching sessions are not the same as mental health therapy / counseling, and are not considered an eligible medical expense. You should not use your HSA/FSA funds to pay for it.

No topic is off the table.

Where Are You Located?

Currently we serve clients virtually via telehealth as well as meeting in-person at six different locations around the region. When an in-person meeting is scheduled, you may choose where to meet. We see clients in Ballard, Mountlake Terrace, Kirkland, Bellevue, Renton, and Downtown Seattle.

This allows us the flexibility and low overhead to run lean, keep costs low, and pass on the savings to our clients.

In the near future we intend to open a brick-and-mortar location, which will also allow us to expand our services further!

How Can I Get in Touch?
